The policy document for Blue Cross Blue Shield's covered transgender care requirements and information!
Blue Cross Blue Shield does also have a specific Gender Care customer service department, but it doesn't have an external number (I guess to prevent harassment) so you call the regular customer service line on your med card and ask them and they'll transfer you.
I've done so a few times now. They can help find in network providers that do specific things like therapy and surgeries.
